Each form must first be downloaded to your computer, before filling in the relevant form fields. Once you have completed the form, press on the submit button. Choose to open your default (or other selected) email application. Once the mail application has opened, you may either send the mail (with the attached approval form) as is, or you may add any other attachments or comments as needed.
